BURIAL ASSISTANCE PROGRAM
- What is the Burial Assistance Program?

- It is a death benefit given to member-consumer-owners (MCO) of INEC
- What are the objectives of this program?

- To instill the sense of belongingness of the member-consumer-owners to the coop
- To update the membership and consumership records of the coop
- Who is entitled to the INEC’s Burial Assistance Program?

- INEC member-consumer-owner who has an “active account” with INEC. Active account means that the account is presently connected or if it is disconnected, it has not remained dormant for a period of six (6) months
- Who are considered beneficiaries of the deceased member-consumer-owner?

- The legal heirs of the deceased MCO based on the following order of precedence:
- Legal Spouse
- Children
- Parents
- Brothers and Sisters
- The legal heirs of the deceased MCO based on the following order of precedence:
- How much is the burial assistance?

- Php 5,000.00 per MCO who are seventy (70) years of age and below
- For MCOs who are above seventy (70) years old, the amount will be graduated in accordance with the length of membership with the cooperative
Number of years as member Amount Due
2 years and below Php 2,000.00
3 years to 4 years Php 4,000.00
5 years and above Php 5,000.00
- What are the requirements in claiming the burial assistance?

- NSO-authenticated Death Certificate of the deceased member-consumer-owner
- NSO-authenticated Birth Certificate of the deceased member-consumer-owner
- Certification from NSO in the absence of Birth/Death Certificate
- Original copy of proof of relationship with the deceased (e.g. marriage contract, birth certificate, etc.)
- Photocopy of two (2) valid IDs (e.g. Driver’s License, SSS/GSIS ID, UMID, etc.)
- Incident Report (in case of accident)
Note: Additional documents may be required as need arises depending on the claimant
- Where can the beneficiaries of the member-consumer-owners apply for this benefit?

- Claimants can apply at the Consumer Welfare Desk of their respective Zone Offices, fill up the “Application for Burial Assistance” and submit the requirements

Zone III Brgy. 4 Nalupta, Batac City
Zone IV Brgy. Masikil, Bangui

- What are the disqualifications to INEC’s Burial Assistance Program?

- Juridical membership
- Outstanding account of more than 60 days
- Self-inflicted deaths
- How many times can it be availed of?

- One availment per account per year
- What is the prescription period in claiming burial assistance benefits?
-
- INEC shall accept application with complete requirements within 60 days from the death of the member-consumer-owner
