Steps in Applying for a Service Connection
1. Go to your respective area office.
2. Attend the Pre-Membership Orientation (PMO).
3. Secure all the requirements.
4. Fill-up necessary forms.
5. Pay the fees

Requirements:
Application for Membership (2 copies)
– 1×1 ID picture
– Pre-Membership Orientation Certificate
From Local Government Unit:
– Electrical Permit
– Certificate of Final Inspection/Completion
– Electrical Sketch – 1-9 outlets or 4kw below
– Electrical Plan – 10 outlets or kw & above
– Application for Temporary Service Connection (for temporary connection only)
Application for Electric Service (2 copies)
– Properly accomplished forms
– Names of (2) nearest neighbor with corresponding account numbers
– Tapping pole number (if missing prepare sketch)
– Location Plan
– Schedule of Load (to be filled-up by the Person-In-Charge of Installation)
– 1×1 ID picture
– Valid IDs (SSS, Pag-ibig, Driver’s License, etc.)
– Barangay Certification
– Community Tax Certificate/Residence Certificate
– Service Connection Agreement
– Notarized Certificate of Undertaking (for tenants, contractor, caretaker)
– Application for Membership and Electric Service Connection
– Temporary lighting contract (for temporary connection only)
